Policies and Procedures

What are the fees for follow up consultations?

New Patient consultation: $185 (1 hour - 1 hour 15 mins)
1 hour: $85
45 minutes: $70
30 minutes: $60
15 minutes: $45

Payment

  • Payment is due at time of consultation
  • Methods of payment are: Personal Check, Cash, Visa, MasterCard, American Express or PayPal.
  • All consultations are time recorded from the time the appointment begins; you will only be billed for the actual time used.

Insurance

Because we do not bill insurance companies, it is up to each individual to determine whether their insurance plan provides coverage for consultations, supplements and lab tests. Per your request, we will provide a superbill to you that lists CPT and diagnosis codes that you may submit to your insurance company. Our superbills provide the codes for consultations and lab tests. Most insurance companies will not reimburse for supplements. Please note that we cannot guarantee reimbursement.

Appointments

  • Follow-up phone and in-office consults may be scheduled in 15, 30, 45, or 60-minute blocks of time.
  • We encourage you to book your appointments at least one week in advance.

Lab Tests

  • The results of your lab test(s) will be sent to our office 10-14 days after mailing your specimens to the lab.
  • Sherri Taylor will evaluate the results. After evaluation we will contact you to schedule a follow-up appointment.

Phone Patients

  • Sherri Taylor will call you at the time of your scheduled consultation. She strives to operate on a precise schedule, so please keep your phone line clear.
  • If paying by credit card, the card number is required to be on file with our office prior to the consultation. Checks are accepted if received prior to the consult.
  • As a courtesy to you, our office will email you to confirm your appointment 1 day in advance.

Cancellations

If you are unable to keep your scheduled appointment, you must notify our office a minimum of 24 hours before your scheduled time or you may be charged for that appointment.

Product Orders

Due to the vast amount of supplements available it is impossible to keep all items in stock. Please consider this and allow adequate lead time to place and receive your supplement orders.

Orders are shipped within 24 hours of receipt of product unless otherwise stated. (Saturdays, Sundays, and holidays not included.)

Shipping

Normally all orders are shipped by regular Ground UPS or Priority mail via USPS.

Returned Products

  • Pre-approval is required on all returns!
  • Refrigerated items cannot be returned.
  • 15% restock fee of purchase price less shipping and handling may be refunded on unopened and non-refrigerated items.
  • No supplement returns will be accepted after 30 days on all regularly stocked items. Special orders cannot be returned!
  • Prepaid tests can be returned for credit within one year of purchase.

Important Notes

  • Sherri Taylor is not a medical doctor; she does not service medical emergencies. If you have a medical emergency, you must contact your primary care physician or dial 911.
  • Please contact the office if you are not clear on any of our policies or procedures.
